Information access and privacy

If you have a request for access to general records, a request for access to personal information, a request for correction to personal information or a request for access to personal health information (clinical records; release of personal health information; confirmation of birth, hospital stay or death), you can submit your request online or contact the Access to Information and Privacy Protection Office.  

This applies if the request is for you, a loved one or a patient (for a lawyer’s office, doctor’s office or other).

To modify, confirm, book or cancel an appointment  

To send a medical referral to a clinic or department or if you have to reschedule or cancel an appointment, please contact the clinic or department directly.

You will find a complete list of the clinics and department here, or under the Receiving Care tab in the main menu. Then, click on the clinic or department you wish to contact. The contact information is provided on the page of each clinic or department.

 

To make an online payment and find out about fees

You can pay a bill from Hôpital Montfort online.

If you have a question about a bill, you can contact the Finance Department at 613-746-4621, extension 2127.

If you would like to find out about the fees for patients who are not covered by the Ontario Health Insurance Plan (OHIP), go to Visiting and Hospitalization Expenses.

 

To use the Patient Portal

As part of the move towards electronic health records, the Patient Portal will allow you to see your records 24/7, no matter where you are.

For now, you can only register for the Patient Portal when you come to Montfort for an appointment, a visit to the Emergency Department or a hospital stay. Our registration staff will ask you if you wish to access the Patient Portal. Consult this page to find out how to log in. 

With your account, you can access, among other things: 

  • your lab reports and results
  • your medical imaging reports
  • your consultation reports
  • your upcoming appointments 

If you did not create an account while you were at Montfort and would like to do so, contact XXX-XXXXX.

If you wish to reach a physician at Hôpital Montfort, you can find the contact information for their office in the Physicians’ directory.

 

Parking and security

For any questions regarding parking procedures, you can consult the "Parking" page.

For any issues related to parking or security, you can write to Security Services at cartesacces@montfort.on.ca

For any situation that compromises security and requires immediate attention, call 613-746-4621, ext. 2634.

  2. speak to the person in charge of the unit concerned, or, if you are still dissatisfied,

What will happen next?

  • Within two business days, you will receive an acknowledgement of receipt for your message
  • We will analyze the situation and make the necessary follow-ups with the departments concerned
  • Within 14 working days following the acknowledgement of receipt, we will share with you the results of our analysis and the recommendations put in place, if any. We will ask for your feedback and if you have any other suggestions
  • We will then communicate these recommendations to the appropriate teams to ensure the improvement of care and services at Hôpital Montfort

  • If you agree, we may publish your message anonymously in the Journal Montfort
