Geriatric Assessment Clinic
Our inter-professional team evaluates patients with a history of falls, loss of balance or fractures. After each visit (lasting approximately 1.5 hours) the team discusses its recommendations with the patient.
We accept referrals from physicians, health care professionals, patients or their caregivers.

Diverse services are offered to our geriatric patients of 65 years old or older in order to maximize their recovery, to maintain their functional level and to improve their quality of life.
At our clinic, a geriatrician evaluates patients with multiple geriatric syndromes, with particular attention to memory disorders. If necessary, we have access to other members of the interprofessional team such as physiotherapists and occupational therapists.
Please note that a medical recommendation is mandatory.

Geriatric emergency management (GEM)
Geriatric emergency management nurses are a key component of the emergency services continuum. They provide vulnerable seniors with access to appropriate services, thus maximizing their functioning capacity, autonomy and quality of life. Geriatric emergency management involves an inter-professional approach that supports comprehensive assessments and the planning and coordination of care addressing the needs of high-risk, vulnerable seniors who require complex medical, functional and social support.
Geriatric Emergency Management (GEM) monitoring clinic
Our clinic performs regular and more detailed geriatric assessments for patients seen in the emergency department the previous week due to acute problems. We develop treatment plans that include in-hospital monitoring, together with family physicians and/or other specialized geriatric services. We remain in direct contact with family physicians by faxing consultation notes to the family doctor’s office.

Access
Only geriatric emergency management nurses may refer patients to our clinic.
This clinic treats patients who have suffered one or more falls. It also works with patients who have unstable gait, dizziness or serious injury.
This clinic is only offered at the Aline-Chrétien Health Hub.
