Montfort recognized for best practice in remote care monitoring
Ottawa, June 12, 2025 - We are proud to announce that Montfort has received official recognition from the Health Standards Organization (HSO) and Accreditation Canada for a healthcare best practice: its Enhanced Care Telemonitoring program.
This model of care allows patients to better manage their condition at home, improves patient satisfaction, reduces emergency room visits and readmissions, and strengthens the capacity of the healthcare system.
“This recognition celebrates not only an exemplary initiative, but also a collaborative, bold and innovative approach that is transforming the patient experience,” says Dominic Giroux, President and CEO of Hôpital Montfort.
This innovative program, developed in partnership with community paramedics from Ottawa and the United Counties of Prescott and Russell, Home Health Ontario and the Ontario Archipelago Health Team, provides 24/7 clinical monitoring for the first 14 to 30 days after hospital discharge for patients with chronic conditions such as heart failure and diabetes.
It reflects the fundamental principles that guide Montfort's activities: quality and patient safety, continuous improvement, data-driven leadership and solidarity with our communities and partners.
This is the 11th best practice recognized by HSO at Montfort since 2013.
