Montfort Ottawa West Community Family Health Team

Montfort will soon open a new primary care clinic for Francophones in the West End of Ottawa at the Maison de la francophonie d'Ottawa (2720 Richmond Road). The Montfort Ottawa West Community Family Health Team aims to gradually take on 6,725 patients by December 31, 2027.

The clinic will initially prioritize serving:

  • Francophones; 
  • who do not currently have a primary care provider;
  • who are enrolled in the Health Care Connect program; and 
  • who live in neighborhoods with postal codes beginning with K2R, K1R, K1S, K1T, K1V, K1Y or K1Z.

Until the clinic opens, Montfort asks the public not to contact the hospital or its partners to register for the new service. Individuals wishing to be served by the Montfort Ottawa West Community Family Health Team are invited to register for the Health Care Connect program to eventually be enrolled.

This new service is part of the latest announcement by the Ontario Ministry of Health regarding the creation and expansion of primary care teams, as part of Ontario’s Primary Care Action Plan. It is made possible by a new annual provincial investment of $2.4 million.
