The Foundation’s Confidentiality Policy includes clauses concerning methods for archiving, using, eliminating and transmitting personal information. Other clauses concern the application of privacy safeguards under the Personal Health Information Protection Act.
This section provides a general outline of the Montfort Hospital Foundation’s confidentiality policy.
- In funding matters, the Montfort Hospital Foundation may obtain, use or disclose personal information on the health of individuals with their express or implied consent only. Furthermore, this information must be limited to the individual’s name and mailing address.
- The Foundation may collect personal health information from Montfort Hospital during fundraising campaigns conducted for a charitable or philanthropical purpose consistent with the activities of the recipient (Montfort Hospital). It may therefore obtain your name and address to inform you of its various activities. However, you can ask to have your name removed from this list at any time.
- The Montfort Hospital Foundation agrees to refrain from renting, selling or trading personal information.
