For all babies born at Montfort
Your baby’s birth is registered online with Service Ontario in two stages. First, a Family Birthing Centre clerk starts the registration process a few days after your child’s birth. It then becomes your responsibility to register your newborn after you return home with the same Service. Your newborn is registered by the FBC clerk under the mother’s family name, as it appears on her health card. Under the provisions of the Vital Statistics Act, all children born in Ontario must be registered within 30 days following their birth.
At the same time, the online service allows you to apply for your newborn’s birth certificate, social insurance number and Canada Child Benefits. Note that charges apply to birth certificate applications.
You will receive an information card entitled, "Newborn Registration Service” during your hospital stay. For more information about registering your child’s birth, call 1-800-267-8097 (toll-free).
After a birth outside Quebec, parents living in Quebec must apply to the Régie des rentes du Québec to obtain Child Support payments.
Health cards for newborns
You will be given a temporary health card for your newborn before you leave the hospital. The official health card will arrive by mail in about 8 weeks. Be sure to complete and hand in your newborn’s health card application to one of our clerks in admitting or in the Family Birthing Centre before you leave the hospital.
You must obtain a health card registration form from the Régie de l’Assurance maladie du Québec either in person at one of their offices or by calling 1-800-561-9749. Once you receive your newborn’s birth certificate, fill out the registration form and mail it in along with the original copy of the birth certificate (which will be returned to you). Your newborn is covered under either parent’s health card until his or her own card arrives.

Hôpital Montfort’s Access to Information and Privacy Office can issue you a birth confirmation. To determine whether you need this document or for more information, please drop by Room 00 D135 (Level “00,” Wing D) or call 613-748-4903 between the hours of 8 a.m. and 4 p.m. Monday to Friday. Someone will gladly answer your questions and advise you of the fees and procedure involved.
The Newborn Club
Join our Newborn Club by making a $100 donation to commemorate the day of your child’s birth. A footprint with your baby’s name and birthdate will be placed on the Newborn Club Wall at the entrance of the Family Birthing Centre.
